Health Care Transition: It’s a Team Effort

In this session we will define Health Care Transition, discuss why it is important and identify practical tools to help improve the process for youth and their families.
Quondalynn Rainey is employed by the Maternal and Child Health Section of the Georgia Department of Public Health and serves as the Program Coordinator for the Children’s Medical Services (CMS) Program. This role primarily focuses on exploring, researching, implementing and sharing evidence-based strategies that will improve successful transition not only within the public health program, but throughout the state for professionals, families and youth both with and without special health care needs. When not working, Ms. Rainey enjoys spending time with her teenage daughter helping to build her confidence, resiliency and self advocacy as she continues to learn how to turn her challenge of Dyslexia and Auditory Processing Disorder into a strength.
